Near Miss Reporting

Use this form to report any near misses or concerns that you come across. You don’t have to tell us who you are but it many circumstances it is helpful if you do. Information entered into this form is sent to an email address that is monitored by members of the Safety Committee.

If you are concerned about confidentiality, you can use the anonymous reporting form, which will send a message to the University Health and Safety Service without revealing who you are.
