Forms

All forms can now be accessed in the Safety section of Chemistry SharePoint site, in the ‘Documents‘ tab.

New Starters

New starters need to complete a Personal Health and Safety Record (PR1). This form records information about the worker, the project and the training needs. The Personal Health and Safety Record is an electronic form – please read the guidance notes (below) before using it for the first time.
In addition to the forms here, the Safety Policy needs to be read and signed (the form is in the Chemistry Hub), and a Display Screen Equipment (DSE) assessment completed (accessed via the ‘Learning’ section in your Oracle account – you will need to search for ‘Workstation Assessment’).

A new PR1 form is required whenever there is a significant change in project details. PR1 forms no longer have a review date, but ‘active’ forms require annual confirmation by email that the details are still valid. This email should be sent to [email protected] by the named researcher or worker on the form using the email address given when the form was completed, cc the named supervisor.

Display Screen Equipment (DSE)

DSE assessments should be completed either:

  • when there is a significant change in IT configuration, e.g. after the purchase of a new workstation or starting work in the Department for the first time; or
  • every two years.

Training, and the DSE assessment itself, can be found in your Oracle account – you will need to search for ‘Workstation Assessment’.

Project Risk Assessments

This form is for the assessment of commonly used processes and protocols within Chemistry laboratories and should be used in conjunction with the New Starter (PR1) forms. Completed forms should be stored by the Researcher and Supervisor and be producible if requested. A central space for storing electronic copies of documents will be made available in the near future (details will follow). Reference to project-based general-risk assessments (forms CRA1/CRA2) may be made in laboratory notebooks using the user-generated reference for the assessment; in these circumstances, a copy of the project risk assessment should be affixed to the cover of the laboratory notebook.

Project COSHH Assessments

This form is for assessing the hazards and risks associated with commonly used processes and protocols within laboratories in the Chemistry department and should be used in conjunction with the New Starter (PR1) and project risk assessment (CRA1) forms.

COSHH Assessment of Single Chemicals

This form is for the detailed assessment of a single chemical (hosted externally by UHSS).

Individual COSHH Form

Radiation

Forms and information for ionising (X-ray systems, sealed sources) and non-ionising (EMF, lasers, UV) radiation (hosted externally by UHSS).

Visitor Form

This form contains the procedure for the reception of academic visitors and documents details of the arrangement.

Permanently Running Equipment

If no cards are available in the COSHH room, use this in the short term.
